For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, the undersigned (the “Model”), hereby grant to Lensof Media, its licensees, successors, and assigns (collectively, “Lensof Media”), the irrevocable, perpetual, worldwide, royalty-free right and permission to capture my image, likeness, voice, performance, and biographical information (collectively, the “Recordings”) through any means, including but not limited to photographs, video recordings, audio recordings, and live streaming, during my participation in any event (the “Event”) occurring between [Start Date of Event or Signing Date] and [End Date of Event or One Year from Signing Date], and for any future dates within one (1) year from the date of signing this release.

As the sole compensation for my time and consideration in connection with this release, Lensof Media will provide me with a selection of the captured images or video recordings in a digital, online format. No physical prints will be provided.

This grant of rights includes, without limitation, the right for Lensof Media to use, reproduce, modify, adapt, publish, distribute, transmit, display, perform, create derivative works from, and otherwise exploit the Recordings, in whole or in part, in any and all media, whether now known or hereafter devised, for any purpose whatsoever, including but not limited to promotional, advertising, marketing, educational, commercial, and non-commercial purposes. This includes the use of the Recordings for creating AI-generated images, training AI models, and any other current or future applications of artificial intelligence.

I understand and agree that Lensof Media shall have the sole and exclusive right to own and control the copyright and all other rights, title, and interest in and to the Recordings and any derivative works thereof.

I hereby waive any right to inspect or approve the Recordings or any use thereof. I understand that Lensof Media makes no guarantees as to the specific number, timeframe for delivery, or aesthetic quality of the photos or videos that will be provided to me.

Notwithstanding the foregoing, I, as the Model, am granted a non-exclusive, non-transferable right to select a reasonable number of photographs from the Recordings for my own non-commercial use, provided that all such uses shall include the attribution “Photo by Lensof Media” clearly displayed and a tag to Lensof Media’s social media account (e.g., @lensofmedia) on all social media posts where the photograph or any derivative work is used.

I hereby release, discharge, and agree to indemnify and hold harmless Lensof Media, its officers, directors, employees, agents, licensees, successors, and assigns from any and all claims, demands, actions, causes of action, damages, losses, expenses, and liabilities of any kind or nature whatsoever, whether known or unknown, arising out of or in any way connected with the Recordings or the use thereof by Lensof Media.

I represent and warrant that I am of legal age and have the full right, power, and authority to grant the rights and permissions herein. If I am under the age of eighteen (18) years, my parent or legal guardian has also signed below.
